I am trying to configure a mixed environment using both IPv4 and IPv6 ... looking at the rfc IPv4-mapped addresses are written as follows:

  ::FFFF:<IPv4-address>

Is there a reliable howto/doc that I can refer to accomplish this ?

Ok, self answering with my findings... there is no special configuration required and ipv4 mapped ipv6 address is automatically generated with the getaddrinfo() function.

As a example, try ssh'ing to your ipv4 mapped localhost as:

# ssh  ::ffff:127.0.0.1 -v -l root
OpenSSH_5.3p1, OpenSSL 1.0.0-fips 29 Mar 2010
debug1: Reading configuration data /etc/ssh/ssh_config
debug1: Applying options for *
debug1: Connecting to ::ffff:127.0.0.1 [::ffff:127.0.0.1] port 22.
debug1: Connection established.

Any ideas on what this might mean ?

ssh ::ffff:127.0.0.1 -v -l root OpenSSH_6.6.1, OpenSSL 1.0.1j-fips 15 Oct 2014 debug1: Reading configuration data /etc/ssh/ssh_config debug1: /etc/ssh/ssh_config line 51: Applying options for * debug1: Connecting to ::ffff:127.0.0.1 [::ffff:127.0.0.1] port 22. debug1: connect to address ::ffff:127.0.0.1 port 22: Connection refused ssh: connect to host ::ffff:127.0.0.1 port 22: Connection refused
