Every time I reboot it goes to emergency mode

My computer (fedora 22) is booting into emergency mode and I need to ctrl-D to move on each time. It works for some time, but after working for about 1-2 hr, it again freezes. The mouse pointer works, but it doesn't work. So again when I restart the machine, it goes back to emergency mode. I ran smartctl, and the test results are as follows.

I see there are some read errors on the disk itself. It could be that you're experiencing hard disk failure and that's the reason why fedora sometimes goes into emergency mode.

Additionally, you're running EOL version of Fedora which isn't officially supported anymore, it is recommended to upgrade to a latest release which is Fedora 25 to stay current.
